Whistleblowing Report Form

The identity of the reporting person will be treated confidentially throughout all stages of the investigation.

Reports submitted in good faith that are subsequently found to be unsubstantiated will be closed without notifying third parties.

Reports submitted anonymously or by persons who cannot be identified may be excluded from investigation. Reports will be investigated in accordance with the procedural provisions of the internal whistleblowing policy.

Reports submitted in bad faith may result in civil and criminal legal consequences. The protection provided by the internal whistleblowing system does not extend to knowingly false reports.
